چکیده

Cardiogenic shock following myocardial infarction remains a significant clinical challenge. Newly reported 30-day mortality rates remain in the 40% range and are not affected by the use of intra-aortic balloon counterpulsation (1). Few cardiac conditions are more deadly. The major exception is patients who suffer cardiac arrest with their ST-segment elevation myocardial infarction (STEMI) and, once resuscitated, manifest cardiogenic shock. Garot et al. (2) from Paris have shown that such patients have a 6-month mortality of 67%. In this post–cardiac arrest population, multiple stepwise logistic regression analysis showed the

Primary percutaneous coronary intervention for unprotected left main disease in patients with acute ST-segment elevation myocardial infarction the AMIS (Acute Myocardial Infarction in Switzerland) plus registry experience.

OBJECTIVES This study sought to assess outcomes in patients with ST-segment elevation myocardial infarction undergoing primary percutaneous coronary intervention (PCI) for unprotected left main (LM) disease. BACKGROUND Limited data are available on outcomes in patients with ST-segment elevation myocardial infarction undergoing LM PCI. METHODS Of 9,075 patients with ST-segment elevation myoc...

Resuscitation Science Therapeutic Hypothermia After Out-of-Hospital Cardiac Arrest Evaluation of a Regional System to Increase Access to Cooling

Background—Therapeutic hypothermia (TH) improves survival and confers neuroprotection in out-of-hospital cardiac arrest (OHCA), but TH is underutilized, and regional systems of care for OHCA that include TH are needed. Methods and Results—The Cool It protocol has established TH as the standard of care for OHCA across a regional network of hospitals transferring patients to a central TH-capable ...

Clinical outcomes in a community-based single operator coronary interventional program.

BACKGROUND Physicians who perform fewer than 75 coronary interventional procedures annually and centers where fewer that 200 procedures are performed annually are considered to be low-volume and likely to have higher procedural risk and suboptimal outcomes. METHODS This was assessed in a retrospective analysis of clinical outcomes in 559 patients who underwent multi-device coronary interventi...

Comparison of in-Hospital Outcome after Percutaneous Coronary Intervention Between Patients with Anterior Myocardial Infarction and Those With or Without Ramus Intermedius

Introduction: Primary percutaneous coronary intervention (PPCI) is used for the treatment of ST segment elevation myocardial infarction (STEMI). Anterior STEMI is associated with adverse outcomes, and it is possible that the presence of ramus intermedius (RI) would inversely affect the outcome. This research involved the evaluation of the influence of RI presence on clinical ou...
